First Visit

Thank you for choosing Lexington Oral Surgery! We look forward to welcoming you to your first visit during which we will review your medical history and discuss your diagnosis. After addressing your concerns and expectations, we will recommend the appropriate surgical and anesthetic procedures and provide you with a custom, effective treatment plan.

Note: Most first visits are sufficient for us to determine a treatment plan. Occasionally, a complex medical history or an extensive treatment plan may require additional appointments before treatment is started.

For your convenience, you can download and print your forms and fill them out and bring them with you. Please arrive 15 minutes before your scheduled appointment time if you have completed the forms. Otherwise, please arrive 30 minutes ahead of your appointment time to allow time to complete all your paperwork.

Please bring these items with you to your appointment:

• Your dental and medical insurance cards, if applicable
• Photo ID
• Current list of medications
• Surgical referral, if applicable
• Recent x-rays, if less than one year old

***Under 18? ***
All patients under the age of 18 must be accompanied by a parent or legal guardian at the first visit. Legal guardianship paperwork or Power of Attorney documentation must be provided at this time.
